Terms of Service
These Terms govern your use of lqwellnessclinic.us and the services of L&Q Wellness Inc., a Florida corporation at 9201 NW 114th St, Unit 6, Hialeah Gardens, FL 33018. Clinical care is also governed by the consent forms you sign at registration.
1. Our services
L&Q Wellness provides wellness exams and physicals, medical weight management, IV hydration and vitamin therapy, laboratory testing, chronic condition management, and nutrition and lifestyle coaching. Services are delivered by Florida-licensed clinicians under physician supervision as required. You pay for the visits, tests, infusions, and programs you receive, through your insurance and/or as self-pay.
2. Not for emergencies
We are not an emergency facility. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Do not use our website, email, or contact form for urgent medical needs.
3. Website information is not medical advice
Content on this website is general information only. It does not replace an evaluation by a qualified clinician, and using the website does not create a provider–patient relationship.
4. Appointments
- Please arrive on time with your ID, insurance card, and medication list.
- Please give at least 24 hours' notice to cancel or reschedule.
- We may decline to perform elective services, such as IV therapy or weight-management medication, if a clinician determines they are not medically appropriate for you.
5. Payment
- Copays, coinsurance, and deductibles are due at the time of service.
- You are responsible for charges your insurance does not cover. We will tell you before providing services we expect not to be covered.
- Self-pay patients receive a good-faith estimate before scheduled services. If the final bill is substantially higher than the estimate, you may have the right to dispute it under the federal No Surprises Act.
- Refunds and credits are handled under our Refund Policy.
6. Telehealth
Telehealth visits are available to established patients located in Florida at the time of the visit. You are responsible for a private location and a working connection. Clinicians may require an in-person visit when necessary for safe care.
7. Patient responsibilities
You agree to provide accurate health and insurance information, follow your agreed care plan or tell us if you can't, and treat staff and other patients with respect. We may end a patient relationship with written notice and reasonable time to find another provider for abusive behavior or repeated serious non-adherence to clinic policies.
8. Limitation of liability
Nothing in these Terms limits liability that cannot be limited under Florida law. To the extent permitted by law, L&Q Wellness is not liable for damages arising from use of the website or reliance on general website content.
9. Complaints and disputes
Please raise concerns with our practice manager at (305) 558-4192 or support@lqwellnessclinic.us. We acknowledge within 1 business day and respond in writing within 7 business days. Clinical concerns are reviewed by our collaborating physician. These Terms are governed by Florida law, with venue in Miami-Dade County, Florida. You may also contact the Florida Department of Health regarding licensed practitioners.
10. Changes
We may update these Terms; the effective date above reflects the latest version.
